船长们看过来,这是亚洲PSC船长方面缺陷100项


◆  本文记录的是2015年至2017初这段时间内,PSC检查关于船长方面或者是船长也占一部分的缺陷。

◆  主要为东京备忘录部分,借鉴以往亚洲各港口国检查的记录,会有利于我们接下来的工作

◆  在某些国家可用小费来解决的,不在本文中体现。


以下为100项的汇集:

【1】EMERGENCY TOWING BOOKLET-NOT INCLUDESHIP'S SPECIFIC INFORMATION(E.G. SHIP'S PARTICULAR BOW AND STERN LAYOUT, S.W.LOF CONNECTION POINT FAIR LEADERS, BOLLARDS, ETC) ,Emergency Towing Booklet中资料不全(连该轮particular都未填)

【2】Some officers o/b not conductedfamiliarzation with SOPEP as per requestion of ship manual.(甲级船员未阅读SOPEP并记录

【3】Fire control plan, Cargo securing manualnot approved by ship class (N.K) 没有经过目前船级社认可      

【4】Loading Manual(Including Trim &Stability)没有经过目前船级社认可Trim & stability booklet not approved by ship class (N.K)

【5】Damage control plan 没有经过目前船级社认可  Damage control plan not approved by shipclass (N.K),更改船级社,没有及时请新船级社对以上资料进行重新认证.

【6】Load line mark on board not confirm withship certificate and regulation  载重线标志与LOAD LINECERTIFICATE不符合规则

【7】Development of Plan for shipboardoperations Checklist was not implemented航行检查表纪录不完整

【8】no evidence showing lash every emg.steering drill carried out within last 3 months(没有证据证明最近3个月有进行紧急舵演习)

【9】 EPIRB on board is notlisted in Shore Base Maintenance and Station License(EPIRB更新, 但未重新记载于 Shore-Based maintenance 证书上).

【10】CSR NO4 WAS RECEIVED BY SHIP MISSING FILLIN DATE & SIGNATURE (RECEIVEDIT IN 2009)(CSR NO4船上接受时没填日期和签名

【11】GMDSS LOGBOOK,MISSING MASTER SIGNAURE FROM29-JAN-2013 TO 04-FEB-2013(电台日志从15年1月29日到15年2月04日无船长签名

【12】No verification of SSO in the visitor logbook after completion of each page.更位的保全记录簿没有大副及船长之签署栏位

【13】碰撞产生的船体损伤Deficiency: Bulwark(stbd-side)on forecastle deck-damaged(abt 5m)

【14】碰撞产生的船体损伤Deficiency:Forecastle deck (stbd side)-buckled partly and cracked(abt20cm)

【15】Gmdss log book 无遇险警报,紧急信息等相关记录,AMOS 等常规电报无须记录在该记录本上

【16】Navtex 在沿岸航行没有使用,检查期间处于关闭状态

【17】The bow and stern of ship in emergencytowing booklet not the same with the ship's arrangement           紧急拖带手册内容,船艏及船艉图示部份与船上实际情况不符合

【18】No training of 2012 for Emergency TowingBooklet on board ;船上无2012紧急拖带训练记录

【19】PSCO询问如果MF/HF与VHF发生误报警该如何取消,二副已当场进行正确解释说明,但是PSCO认为应有一个操作程序书在放在MF/HF与VHF旁边备查. Nofalse alert procedure placed beside MF/HF

【20】Funnel door from engine room does notprovide means of escape for the engineers(机舱Funnel出口(F-DECK)门在外侧上锁,内部人员无法打开

【21】启动紧急救火泵压力上不来Deficiencystatement:Emergency fire pump failed to pressure firemain line during testing

【22】3/E 适任证书没有本人签名 Deficiency: 3/E NATIONAL CERT. MISSING SIGNATURE OF HOLDER

【23】球鼻首因撞击部分受损。some part ofbulbous bow damaged(small)

【24】驾驶台未见2013版之IAMSAR manual Vol.III, 只有2008版之IAMSAR manual Vol.III(船长书柜内).Non-conformity:Outdated (2008) edition of IAMSAR manual Vol.III found on board

【25】应急舵操演程序里面没有关于将罗经从驾驶台带到舵机房的任务分配 Emergency steering gear drill procedure did not be mentioned theduty to bring gyro repeater from bridge to steering gear room.                              

【26】计程仪工作不正常没有被报告给船旗国,获得其同意进坞修理speed log out of work did not be reported to SIN administration

【27】船上缺少DNV Class发给之『未经处理之holding tank内污水在航行中不同吃水时排放之航速表』Non-conformity:Lack of maximum rate table for discharge of untreated sewage of holding tank

【28】LARGE MAGNETIC COMPASS DEVIATION (OBSERED)HAS BEEN NOTED,IE,EXCEEDED FROM THE RESIDENT DEVIATION CARD IN USE  记录的观测磁罗经差值超过了“年度矫正磁罗经自差表”的记录

【29】SOPEP 联络点没有更新(The contact point in SOPEP not up to date.)

【30】Ship working language not clearlyspecified in log book  航海日记上未明确标明船舶工作语言

【31】Lifeboats not launched into water formaneuvering during last 6 months as per ship's record  根据船上纪录无法标明在过去6个月有降落救生艇于水面进行操纵

【32】No record sighted onboard as evidence ofSSAS test carried out with flag state as per sc 09 of 2015./没有船旗国的SSAS 测试纪录

【33】No procedure sighted onboard for securityof BNWAS ie action key found along with the panel of BNWAS./BNWAS的钥匙长时间置于面板上

【34】CSR没有更新  CSR notupdate

【35】航次计划值班驾驶员没有签字Previousvoyage plan not read by OOWS as required

【36】船员休息时间中不能包含演习时间(Hour ofwork and rest not included with drill hour as working hour.

【37】FSC官员认为船上焚烧垃圾记录时,应该纪录焚烧前垃圾的代码,不能都记录为焚烧后的代码E (炉灰) (Wrong entries for garbage incineration code. Code E is used forgarbage incineration

【38】海图上面没有转舵点标志,没有平行避险线的标识,没有每段航向速度控制的标志,没有偏航界限的标识。(No information regarding wheel over position, parallel indexing,reduction of speed and off track distance reflected in the voyage charts.)

【39】驾驶员对GMDSS设备的电源转换不熟悉。(Deck officer not familiar with the GMDSS equipment reserve power.

【40】Voyage plan does not indicate wheel overposition, speed reduction & off track distance/ 航路计划上没有标注转向施舵点, 减速提示和允许偏航距离等

【41】Vessel does not have consistent reply, andno measures have taken to address this issue. /船上对于MF/HF DSC 测试没有收到回复没有采取积极的措施 (DSC测试澳洲岸台记录中最近没有岸台回复,不协调, 要求一定都要测试和确认回 )

【42】Navigating officers not able todemonstrate the pages to refer to the publication reflected in the voyage plan./航路计划上参考航海刊物没有标注参考页数

【43】MF/HF DSC transmission test call to coaststation not available for verification/测试信文内容缺乏足够证据显示已测试岸台

【44】Wheel over position, speed reduction &off track distance not reflected in the chart in used. /海图上没有标注转向施舵点、减速提示和允许偏航距离等

【45】Obsolete MF/HF antenna nearby the newlyinstalled MF/HF antenna   废弃的MF/HF 天线靠近新安装的MF/HF天线

【46】Shipboard working arrangement table wasnot identified for every crew onboard and 4 cadets were missing from the table,total daily rest hours were not indicated for crew awareness.船上工作安排表没有包括所有船上人员,缺少4个实习生的工作安排;没有标明让所有人了解的每天总的休息时间。

【47】WRONG SHORE STATION SETTING OF NAVTEX.NAVTEX 岸台设置错误,岸台设置香港而不是广州

【48】ERROR CODE LIST OF S-VDR NOT PROPERLY  POSTED.S-VDR错误警报代码未适当张贴

【49】LRIT OPERATIONAL PROCEDURE NOT PROPERLYPOSTED.LRIT之操作程序未张贴

【50】CREW MEMBERS NOT FAMILIAR PROCEDURESRELATING TO LRIT.  船员不熟悉LRIT操作流程

【51】主机未遵守SMS指示到港前执行倒车测试     DESCRIPTION: M/E STERNNOT TIRED OUT ACCORDING TO SMS MANUAL BEFORE ARRIVAL PORT OF YANGSHAN ON MAY15, 2015.

【52】FPDS 操作程序未涵盖于SMS内且无检查记录  DESCRIPTION: OPERATION PROCEDURE OF FPDS FORLIFEBOATS NOT CONTAINED IN VESSEL SMS DOCUMENTATION AND NO INSPECTION RECORDFOR IT.

【53】救生艇周检最近两月有移动但未记录    DESCRIPTION: WEEKLY INSPECTION NOT CARRIEDOUT AS PER SOLAS REGULATION(LIFEBOATS NOT MOVED FROM THEIR STOWED POSITION INRECENT TWO MONTHS)

【54】2/O's certificate of competency withouthis signature ,    二副的适任证书,本人没有在持有人处签字,船长没有检查到

【55】COMPLAINT PROCEDURE & RLEVENT CONTACTDETAILS ARE NOT MADE READILY COVERAGE TO EACH CREW ONBOARD/MLC CODE中的船员投诉声明和程序未完全发至所有在船船员

【56】大副的休息工作时间与车种记录簿不符 ,船长疏忽检查

【57】船上工作安排表显示没有足够的休息时间 ,且前后不一致

【58】VDR显示错误代码94和98,并且不能复位

【59】Three watertight doors heavy rusted lostwatertightness, including two in bosun store and one in the portside upper deck(三个水密门边缘锈蚀,包含水手长司多间两个及主甲板左舷一个)

【60】固定CO2释放系统释放操作程序与实体标注不对应,操作起来模糊不清   CO2 fire extinguish system release control instruction fail to enablesenior officers and engineers familiarized with operation procedures

【61】左右小艇没有舷边遥控释放操作装置   Both Lifeboats launching appliance, meansnot provided that boat should be visible to the on-deck operator when beinglaunched

【62】VOYAGE PLAN NOT DEFINED POSN FIX FREQUENCYFOR COASTAL AREA AND INCLUDED ESSENTIAL INFORMATION FOR MONITOR HAZARDS,COASTAL POSITION NOT MONITORED  EVERY15MINUTES./ 新加坡到香港的航路计划上未定义沿岸航行的定位频率及未包括基本的监视危险物的信息,沿岸航行定位间隔未按照航路计划的每15分钟定位一次

【63】THREE ANTENNAS FOR RADIO NOT SECUREDPROPERLY/ CORRODED CONNECTION../ 3支RADIO天线未正确固定/连接点锈蚀

【64】MAGNETIC COMPASS (STANDARD) MISSING TWOCORRECTIVE SPHERES/( 标准)磁罗经的2个校正球不见

【65】SHIPBOARD WORKING ARRANGEMENT TABLE WASREFLECTED WITH NO REST ON CREW'S WORKING SCHEDULE AND UNABLE TO ENSURE SHIP'SSTAFF ARE REST PROPERLY/ 此船上工作分配表内,没有显示出船上人员适当的休息时间

【66】中高频交流电转换直流电说明步骤没有张贴  NC description:AC ANDDC POWER TRANSFER OPERATION INSTRUCTION OF MF/HF N/A                                                                            

【67】THE KEY FOR BNWAS SWITCHING ON/OFF NOTUNDER SECURITY CONTROL BY MASTER.   BNWAS开关钥匙没有处于船长控制下

【68】Disposal of cooking oil is not as perCompany’sGarbage Management Plan (i.e. cooking oil was disposed into engine room sludgetank and recorded into both Garbage and Oil Record book.)厨房用油卸岸不符合公司垃圾管理计划

【69】Rest hour didn't record correctly. PSCOconsidered that no matter how long (even just go to E/R to reset alarm), if C/Eand 2/E have been in engine room. The half hour is not intact as rest hour. Thehalf hour should be recorded as working time. Same as the deck department. IfMS and C/O have done their job (i.g. receiving/sending E-mail, doing inspectiontour when sailing or asking the cargo work schedule, meeting visitor whenberthing .etc). The half hour can not be recorded as rest hour.休息时数纪录不正确,FM 0606-2"RECORD OF HOURS OF REST OF SEAFARERS - 2" 必须标明姓名、职务及本人签名,PSCO认为只要轮机长和大管有在休息时间至机舱工作,无论时间长短(即使只是到机舱消除警报),则该时段就不是完整的休息时间,故该时段(0.5hr)应纪录为工作时段。同理,船长若于放洋时有进行收发报或甲板巡视等工作,又或是大副在港时有看水尺、询问值班船副装船进度、接待访客等行为,无论时间长短(即使只有五分钟),则该时段亦不能记做休息时间。

【70】NO.1 battery for P'side lifeboat was lowcapacity, it was not available to support P'side lifeboat' motor engine startmany times 左舷救生艇NO.1电池低储存电量,不能支持小艇马达重复多次启动,PSC官员要求换新.  

【71】ECDIS,OFFICER REPORTED IT IS FORNAVIGATION AND CSE CERT NOT RECORD.船副回答电子海图使用有时助于航行但仅供参考,且船舶安全设备证书没有记录

【72】船员休息时数未能符合公约规定      Nc description:Recordof Rest hours not comply with the Requirement

【73】固定式CO2钢瓶使用10年后没有执行10%的钢瓶静水压力测试   Hydraulic pressure test of fixed CO2 firefighting system(10% of bottles)not carried out after 10 year into use 

【74】固定式CO2连接弯管使用10年后没有换新 flexible hose for fixed CO2 firefighting system not changed at interval of 10 years after put in use

【75】Bridge, Fire detection and alarm system,power supply of battery for control panel fault, repairing in progress noted. 驾驶台的烟雾警报侦测系统电源供应的电池故障,附记修理在进行中

【76】Work/rest hours e.g. all crew membersparticipated safety drills on 12 Sep 2014, but the record indicated some ofthem were recorded as resting during the period of safety drills. 休息时数在某些场合未能正确记录,例如,所有成员皆于2015-09-12参加船上安全操演,但记录却显示某些成员在这段演习期间在休息

【77】2015年10月20日操演纪录中,消防演习设定失火位置为舵机间,当时使用位于舵机间内的应急消防PUMP灭火,PSC认为有矛盾)

【78】SEAFARER'S DAILY RECORDS OF REST HOURS DONOT REFLECT ACTUAL HOURS OF REST, EMERGENCY DRILL, HALF REST PERIODS AND ENGINEROOM ROUNDS FOR UMS SHIP NOT RECORDED AS WORK HOURS 船上工作休息时间记录没有正确记录应急演习时间,记录休息半小时不符合规定及机舱无人值班时巡逻机舱时间应记录

【79】Officers are not familiar with BNWAS “on”mode of operation.驾驶员不熟悉BNWAS操作

【80】除了学生是“训练”以外,按照公约规定,负有保安,安全,防污染有关职责的所有人员都属于值班人员,因此除了学生之外所有在船人员都为值班人员。另外休息时间记录表应能正确清晰反映人员的休息时间分为几段,及最长一段时间记录。大副在码头值班有关“on call”定义模糊。  HOURS OF REST RECORDS FOR WATCH KEEPERS NOTRECORDED AS PER STCW REQUIREMENT(REST HOURS IN ANY 24 HOURS,WEEKLY RESTHOURS)AND C/O HOURS OF REST DURING DAY TIME IN PORTS IN OCT/2014 NOT RECORDEDPROPERLY.(ONLY MARKED ON SCHEDULE ON CALL)                                                                                                               

【81】Bridge, Fire detection and alarm system,power supply of battery for control panel fault, repairing in progress noted. 驾驶台的烟雾警报侦测系统电源供应的电池故障,附记修理在进行中.

【82】According deficiency ship ISM code failure,-ship board operation.-resources and personal;缺失过多致ISM CODE失败。

【83】HE DAU (data acquisition unit) OF VDRFOUND ERROR DURING INSPETION.  VDR之数据采集单元显示Error之警报。                        
【84】SOPEP MANUAL RECORD FOR ANNEX II LIST OFCONTACT POINT NOT UPDATE油污应急计划中收到ANNEX II在收到ANNEX II的清单中没有更新

【85】EIAPP cert for A/E not found duringinspection船上检查时未具备机舱辅机I.A.P.P.证书

【86】Large scale chart for pilotboarding(western boarding ground B) area not available on board. I.e. chartno.4030 or 4031. 船上未配备领港登轮区(WBGB)大比例尺海图,如BA4030或BA4031

【87】Echo sounder AFT sensor unable to indicatethe water depth by digital display unit. 后部之测深仪探测头探测不到水深(液晶面板不能显示水深数值)

【88】COPY OF DOC(ISM) NO LATEST VERSIONONBOARD.船上没有最新版本的DOC(ISM)复印件

【89】船上演习纪录反映出海上演习时所有船员均有参加,无人缺席.  Vessel's drill records(I.e.Abandon ship,flooding drill) reflected all ship crew participated while she wasat sea and have not excluded officers on duty.

【90】AIS RPT(Annual Performance Test) expiredin FEB-2015.  AIS年度测试报告发现于2015/FEB过期

【91】No record of lifeboat lowered andmaneuvered in water for last 3 months found in official log book. 船旗国记录簿之求生灭火操演记录三个月内脱勾水上操作操演没有详细记载

【92】Barometer last calibration done inSep/2012.  船上气压计上次美国NOAA校正日期为SEP/2012。

【93】Crewmember not familiar with shipboardoperational procedure relating to DSC transmission船员不熟悉DSC操作。

【94】Crewmember not familiar with shipboardoperational procedure relating to NBDP transmission 船员不熟悉NBDP操作

【95】Emergency steering operation blocked byseating position应急操舵位置被画上座位标志

【96】Access control and the boarding point notproperly carried out as per SSP Section 5.9.2 for security level 1.梯口管制并未适当地遵守SSP章则5.9.2所规范保全等级1应采取之行动

【97】四月份及五月份有数位船员(船长,两个二副及AB)休息时数不符合规范(休息时间分成3段)且5月份缺当天记录Hour of rest records, numerous occasions for Master, 2/O(TWO) , ABin April and May 2015 not complied with STCW requirement, records not updatedand accurate recorded.

【98】公司需确认船员工作时数是否符合规范以及航行计划和海图修正的确认company not ensure ship's crew comply with the hour of rest andvoyage plan requirement and chart correction requirement

【99】Night order book has columns of true course,gyro course and magnetic course. However, these information have not beenfilled up since opening the night order book.夜令簿未纪录真航向、电罗经航向、磁航向栏位

【100】Master’s night order has mentioned “wideberth” for all passing vessels. However, officers arenot able to explain the meaning in relation to open sea, congested water andwhile picking up pilot. 夜令簿未定义应距来往船舶应保持之距离为何,因此船副无法说明于开阔水域、狭窄水域及接领港时应与其他船舶保持之最小距离
